Consumer Medication Incident Reporting Initiative

The Institute for Safe Medication Practices Canada (ISMP Canada) consumer medication incident reporting and learning website "SafeMedicationUse.ca" was launched in 2010. In early 2019, ISMP Canada reached out to the Canadian Patient Safety Institute and Patients for Patient Safety Canada to collaboratively develop and launch an initiative to increase the quantity and quality of medication incident reports from consumers.

Goal
To receive more information from consumers on what medication errors they were experiencing and what they believe is needed to prevent a similar error from occurring again.

• Prepare an Environmental Scan of the white and grey literature involving consumer medication incident reporting
• Launch an Advisory Committee of patient/family advisors and collaborating organizational partners
• Bring the Advisory Committee together with other partners to complete a SPRINT workshop to develop possible prototypes for attaining the goal
• Take action to implement the recommended prototypes developed at the workshop
