“U” Can Make a Difference - Eliminate Dangerous Abbreviations!

Due to the overwhelming interest in the session held on August 11, 2010, it has been decided to repeat this webinar.

The use of abbreviations, symbols, and dose designations has been identified as an underlying cause of serious, even fatal medication errors.

This one hour webinar identifies strategies and actions to assist in eliminating the use of dangerous abbreviations, symbols and dose designations within your facility. Tim Baron and Celina Colegrave from Alberta Health Services will be joining us to discuss key learnings they have experienced on their journey of implementing changes to support the elimination of dangerous abbreviations.

Learning Objectives:

At the conclusion of this webinar, participants will be able to:

  • Identify abbreviations, symbols and dose designations considered dangerous;
  • Provide incident examples involving dangerous abbreviations;
  • Identify and apply strategies to safely communicate medication orders.

Speakers

Tim Baron, BSc PT, MBA
Tim is a senior practice consultant with the Health Professions Strategy and Practice Division of Alberta Health Services (AHS), with over ten years of clinical and consulting experience. Currently, Tim is the project leader for the AHS Dangerous Abbreviations project, along with involvement in the AHS response to the Falls Prevention and Information Transfer Accreditation Canada Required Organizational Practices.


Celina Colegrave, BSP, ACPR, MSc
Celina graduated from University of Saskatchewan College of Pharmacy in 1984. She completed her Hospital Pharmacy Residency at Lions Gate Hospital in North Vancouver. Her thirst for knowledge led her to complete a Masters Degree in Health Service Planning and Administration at UBC in 1989. Since then Celina has held numerous Management positions in both Vancouver and Victoria before moving to Sunny Alberta. She has started with the David Thompson Health Region in Jan 2005 working on numerous Quality Improvement initiatives in the Pharmacy Department at Red Deer Regional Hospital. Then in Sept 2005 she accepted the permanent position of Medication Safety Officer for the David Thompson Health Region. During her 4 year tenure in this position she led 2 projects for the implementation and evaluation of a Dangerous Abbreviations Policy for the region. Now in her current role as Quality Consultant for Alberta Health Services she is participating on a provincial working group to develop and role out a Dangerous Abbreviations Policy for the province.


Shirley Sabovitch, RegN, BScN
Shirley is a medication safety specialist at ISMP Canada with over 25 years of nursing experience, in healthcare facilities and also in academic centers. Currently, Shirley is involved in medication system reviews as part of the ISMP Canada consultation program, delivery of educational workshops (Root cause Analysis [RCA] and Failure Mode and Effects Analysis [FMEA] across Canada), and responding to general inquiries received by ISMP Canada for the purpose of enhancing medication safety and facilitating knowledge translation.
