Improving the safety of ambulatory IV chemotherapy in Canada:
Results and recommendations from a 2–year exploratory study

The death of a patient due to a fluorouracil overdose, and other similar incidents, highlighted the risks of ambulatory IV chemotherapy. As a follow–up to the root cause analysis of the event, a two–year research project was funded by a number of cancer and safety agencies across Canada. The objectives were to identify safety issues in ambulatory IV chemotherapy in a wide range of environments.
This one hour webinar will highlight the results and recommendations from the two–year pan–Canadian exploratory study on safety issues in ambulatory intravenous chemotherapy.

- Describe the rationale and overall approach to the study
- List 3–4 themes of safety issues identified in the research
- Identify areas of improvement in their own chemotherapy delivery processes

Rachel White, M.A.
Human Factors Specialist, Health Technology Safety Research Team, University Health Network, Toronto
Rachel is passionate about making healthcare safer for patients by making environments, processes and technologies more intuitive for clinicians. She has a keen interest in integrating human factors principles into medication safety initiatives, especially those relating to chemotherapy. Through her research on independent double checking of high–risk clinical procedures, and the study Improving the Safety of Ambulatory IV Chemotherapy in Canada, she has gained a strong understanding and respect for the role that clinicians, especially pharmacists and nurses, play in patient safety.
A graduate of Carleton University in Ottawa, Rachel received an MA in Psychology, specializing in Human–Computer Interaction, from the Human–Oriented Technology Lab (HOTLab).

Venetia Bourrier, BSc., B.Sc. Pharm., FCSHP
Director of Provincial Oncology Drug Program, CancerCare Manitoba
Venetia received her Bachelor of Science degree in Pharmacy from the University of Manitoba in 1985 and a Bachelor of Science in Molecular Biology form the University of Winnipeg in 1981. She is a member of the Canadian Society of Hospital Pharmacists (CSHP), the Manitoba Pharmaceutical Association (MPhA) and the Systemic Therapy Safety Committee of the Canadian Association of Provincial Cancer Agencies (CAPCA). From 2004 to 2008 she acted on the planning committee for the National Oncology Pharmacy Symposium (NOPS). In 2004 she was the recipient of CSHP Fellow Status and CSHP Manitoba Branch Merck Frosst Achievement award. From 2002 to 2004 she was President of Manitoba Association of Pharmacy and was on the task force to develop and review oncology pharmacy practice standards. In 2002 and 2003 she was Chief Administrator for the Pharmacy Examining Board of Canada (PEBC) Qualifying Examination Part II OSCE. Venetia also participates on several advisory boards on oncology topics.
