Attendees of this one hour webinar will learn how Central CCAC implemented Medication Management Support Services and about
the role of the home visiting pharmacist in medication reconciliation and assessment. Outcomes achieved have improved safety for clients by reducing falls, pain symptoms, and avoiding potential hospitalizations and ED visits.
Learning Objectives:
At the conclusion of this webinar, participants will:
- Understand what Medication Management Support Services is in Central CCAC and who is eligible
- Be able to define and quantify medication issues that occur between the hospital and home environment
- Understand key processes that drive success
- Be aware of tools and techniques that can be used to develop and implement a similar program to improve patient safety in their own setting

Mary Burello–Cordovado, RN, BSCN, is Senior Manager, Project Lead, Central Community Care Access Centre. Mary’s 19 years of health care experience includes extensive hospital and community experience. In 2008, her work on the Flo Collaborative was awarded the national 3M Health Care Quality team award, with North York General Hospital. More recently, Mary has advanced Wound Care and the Aging at Home initiatives through her work in Balance of Care and Medication Management Support Services, which was nominated for Ontario’s prestigious Patient Safety Health Achieve award in 2010.

Lisa Sever, RPh, ACPR, BSc Phm graduated from the University of Toronto in 1992 and completed her residency at St. Joseph’s Hospital, Hamilton, ON in 1993. She has held positions at St. Joseph’s and York Central Hospital focusing on the ambulatory care populations. Her experience with the dialysis population allowed her to witness the medication complications that arise when patients cross the continuum of care – home to hospital and back. Lisa joined the Medication Management Support Services at a Kaizen event in August 2008 and immediately saw how pharmacists in the home care setting could benefit patient care. She joined the team as the Lead Pharmacist in December 2008.
