Background
Medication safety, defined by the Institute of Safe Medication Practices Canada (2007), is “freedom from preventable harm with medication use ”. Many studies explore the precipitating factors, incidence, and impact of medication errors in hospitals as these can threaten medication safety for patients and practitioners. Few studies document how practitioners view medication safety in their work processes and practice environment. In this participatory practitioner–guided study, practitioners and front line managers in pharmacy and nursing identified supports for and barriers to medication safety on an acute care hospital unit.
