This one hour webinar will begin with a brief overview of the ISMP Canada Community Pharmacy Incident Reporting (CPhIR) Program in order to provide context for the aggregate analysis. The significance of order entry errors will then be highlighted. Participants will learn about the two main themes of order entry errors identified through the aggregate analysis of 1100 incidents reported to the CPhIR program. Sample reports will be showcased throughout the presentation to provide a deeper understanding of the materials presented. The session will conclude by offering potential system-based strategies that can help alleviate order entry errors and other medication incidents identified through aggregate analyses.
Learning Objectives:
Participants will be able to:
- Recognize the significance of order entry errors in the pharmacy.
- Learn about common order entry errors identified through an aggregate analysis of medication incidents.
- Recognize some of the common causes or contributing factors of order entry errors.
- Learn about potential safeguards that can be put in place to minimize the risk of order entry errors and medication incidents.

Edgard Petrov, B.Sc.Phm. Candidate
Edgard Petrov is in his second year of completing a Bachelor of Pharmacy degree at the School of Pharmacy, University of Waterloo. He is currently on a co-op work term working as an Analyst at the Institute for Safe Medication Practices Canada (ISMP Canada).
In his current position, Edgard is responsible for overseeing the implementation of upgrades to both the Community Pharmacy Incident Reporting (CPhIR) program and the Medication Safety Self-Assessment (MSSA ®) for Community and Ambulatory Pharmacy. In addition he is responsible for aggregate analysis of medication incidents and the preparation of newsletters to disseminate shared learning from medication incident reporting and analysis at ISMP Canada.

Certina Ho, R.Ph., B.Sc.Phm., M.I.St., M.Ed.
A graduate of the Faculty of Pharmacy, University of Toronto, Certina Ho is currently a Project Manager at the Institute for Safe Medication Practices Canada (ISMP Canada) and Adjunct Assistant Professor at the School of Pharmacy, University of Waterloo.
In addition, she has experience in community pharmacy practice and management of internet health content. Certina obtained her graduate degrees in Library and Information Science in 1998 and in Education in 2005. In her current position, Certina is responsible for medication safety initiatives related to community pharmacy practice as well as medication incident reporting and analysis at ISMP Canada.
