This one hour webinar shares the insights gained from an aggregate analysis of medication incidents with an outcome of death that were voluntarily reported to ISMP Canada. Starting from the various death scenarios (endpoints), this webinar describes a step–by–step approach tracing back towards the potential contributing factors. The medications most frequently associated with these tragic incidents will also be discussed using incident examples.
Learning Objectives:
At the conclusion of this webinar, participants will:
- Have a better understanding of the types of medication errors more likely to lead to death
- Be able to recognize some of the potential systems–based contributing factors in incidents with an outcome of death
- Be able to identify potential areas for medication system improvement in their practice settings

Roger Cheng, RPh, BScPhm, PharmD
Roger joined ISMP Canada in 2007. His current responsibilities as a project leader include conducting individual and aggregate analyses of medication incidents voluntarily reported to ISMP Canada.
Roger holds a Bachelor of Science in Pharmacy degree (1999) and a Doctor of Pharmacy degree (2003) from the University of Toronto. Roger has worked in both community and hospital pharmacy settings, including staff pharmacist positions at Princess Margaret Hospital and York Central Hospital.
