None of us likes to think that our doctor, nurse, pharmacist or caregiver might make a mistake with our medicines, but these mistakes can  – and do  – happen. A mistake with medicines, or a problem that could cause a mistake with medicines, is known as a medication incident. Fortunately, most medication incidents don’t cause harm, but some have caused serious harm and even death.

Patients and consumers play a key role in safeguarding themselves and others from potential harm that can result when mistakes involving medicines occur.

This one hour complimentary webinar from staff at the Institute for Safe Medication Practices Canada will provide patients and consumers with information on SafeMedicationUse.ca, a program that gives consumers a way to report medication incidents AND to learn from medication incidents experienced by others. Examples of tips and tools for safe use of medicines will be presented.

Jennifer Turple – Medication Safety Specialist, ISMP Canada

Jennifer is a Dalhousie University pharmacy graduate who has also completed a hospital pharmacy residency. She has over 10 years of frontline acute care experience as both a Clinical Pharmacist in a general cardiology unit and as a Medication Safety Pharmacist. Jennifer is currently at ISMP Canada where she works as a medication safety specialist supporting and coordinating medication reconciliation activities with Safer Healthcare Now! and as an analyst and writer for the SafeMedicationUse.ca program.

Background

ISMP Canada ’s consumer -focused website, www.SafeMedicationUse.ca, was launched with support from Health Canada in March 2010, as a pilot project of the Canadian Medication Incident Reporting and Prevention System (CMIRPS). CMIRPS is a voluntary, confidential program that collects, analyzes and distributes information on medication incidents. The goal of CMIRPS is to prevent harmful medication incidents. The knowledge gained through analyzing reports submitted to CMIRPS is used to make medication use safer. Up until now, most medication incident reports have been provided by healthcare workers, but CMIRPS also needs information that consumers can provide. SafeMedicationUse.ca provides a way for consumers to report medication incidents and to get information about using medication safely.
