Protecting the living - How death investigations contribute to safe medication use

In 2012-2013, ISMP Canada embarked on an extensive national project to use the formal death investigation files of coroners and medical examiners in order to advance patient safety. This rich source of investigative data can provide insight into contributing factors to medication incidents. This one-hour webinar will outline the rationale for the project, briefly describe the methodology of case review and analysis, and share some highlights of the findings. Presentation of selected cases from the project will emphasize important patient safety concepts arising from coroners’ and medical examiners’ data.
Learning Objectives:
At the conclusion of this webinar, participants will:
- Understand the role the coroner or medical examiner can play in medication incident investigation
- Understand the benefits and challenges associated with using data derived from death investigations
- Learn about selected findings of importance from the Coroners’ and Medical Examiners’ Project
- Be able to apply case based learning to enhance safe medication practices in their own practice setting

Julie Greenall, RPh, BScPhm, MHSc (Bioethics), FISMPC
Project Director, ISMP Canada
Julie has more than 30 years of clinical and management experience in pharmacy practice. Julie joined ISMP Canada in 2004 and as Project Director, is responsible for coordinating, supervising and conducting medication system reviews, incident analyses and other consultation projects. Julie holds an active practice pharmacy license and in addition to her work with ISMP Canada, continues to work in a community hospital on a casual basis.
Since joining ISMP Canada, Julie has participated in numerous medication system reviews, root cause analyses of sentinel medication incidents and prospective risk assessment projects. Julie is a co-author of the 2006 Canadian Root Cause Analysis Framework and the revised Canadian Incident Analysis Framework released in 2012, in addition to numerous medication safety-related articles published in healthcare journals. Julie has provided educational presentations and workshops on a variety of medication safety topics across Canada and internationally.

Michael J.A. Hamilton BSc, BEd, MD, CCFP
Consultant and Medication Safety Specialist, ISMP Canada
Michael has over ten years of clinical medicine involvement, with experience in emergency medicine, long-term care, inpatient care, and office practice. He continues his current practice in York Region in family medicine, walk-in clinic care, and long-term care. He currently serves on the Medical Advisory and Therapeutics Committee for the Community and Health Services Department of York Region and has been a member of the Drugs and Therapeutics Committee of a 400 bed tertiary hospital. Michael has also served as an investigating coroner with the Office of the Chief Coroner for Ontario.
His education, his work, and his experience with multiple health care accreditation cycles has given him an interest in procedure and system analysis, healthcare safety, and methods of designing work processes to minimize the risk of error, while ensuring high quality care.
Michael joined ISMP Canada in 2012 and is a member of the medication incident analysis team, participating in the dissemination of safety recommendations, contributing to the investigation of medication errors, and collaborating in research projects with the Institute.

Each Canadian province and territory has an Office of the Chief Coroner or Chief Medical Examiner responsible for investigating deaths from unexplained, unexpected, or unnatural causes. Within the scope of these investigations are deaths associated with medication incidents. In-depth analysis of information from these cases offers unique opportunities to identify underlying factors and generate recommendations to reduce the chances of similar incidents in the future. ISMP Canada has had a formal collaborative relationship with the Office of the Chief Coroner in one province since 2004, and has worked with other Offices on selected cases. In 2012-2013, ISMP Canada embarked on an extensive national project to use the formal death investigation files of coroners and medical examiners in order review the accumulated data in death investigations, to collate the safety recommendations, and to further advance patient safety by sharing learning.
