BPMH Training for Pharmacy Technicians

Understanding the hospital pharmacy technician’s role in the medication reconciliation process.

1-day workshop

This one-day workshop is designed to provide hospital pharmacy technicians with an overview of medication reconciliation.

There will be a specific focus on the importance of the Best Possible Medication History (BPMH) within the medication reconciliation process and participants will learn how to conduct a BPMH interview and document the necessary components of a BPMH.

At the completion of this workshop, participants will be able to:

Explain the role of medication reconciliation as it effects patient care
Understand where and how medication reconciliation takes place
Understand the role of the BPMH at all care transitions
Describe the process of completing a BPMH
Understand the benefits and limitations to the various sources of a patient’s medication information

Cancellation Policy
Notice of all cancellations must be provided in writing at least 7 days prior to the event. A $75 administration fee will apply to all refunds. There are no refunds for registrants who cancel after the cancellation deadline.

Registrants may postpone attendance to a future workshop however a $75 administration fee will apply if notice of postponement is received less than 7 days prior to the event.

ISMP Canada reserves the right to cancel or reschedule workshops. In the event of a cancellation, fees paid will be credited towards a future workshop, or fully refunded.

BPMH Training for Pharmacy Technicians

Kimindra Tiwana, RPh, BSc. Pharm, ACPR

Kimindra joined ISMP Canada in 2010. Her area of focus has been primarily in supporting medication reconciliation initiatives in Ontario and in the development of an iPhone app for patients to store and communicate their medication related information.

Kimindra has been a practicing pharmacist since 2001. She received her pharmacy degree from Dalhousie University, College of Pharmacy and then went on to do a residency with the Queen Elizabeth Health Sciences Centre. The majority of Kimindra’s clinical experience comes from working at the University Health Network - Toronto Western Hospital.
