BPMH Training for Pharmacy TechniciansUnderstanding the hospital pharmacy technician’s role in the medication reconciliation process.
Wednesday, September 18, 2013 This one-day workshop is designed to provide hospital pharmacy technicians with an overview of medication reconciliation. There will be a specific focus on the importance of the Best Possible Medication History (BPMH) within the medication reconciliation process and participants will learn how to conduct a BPMH interview and document the necessary components of a BPMH. At the completion of this workshop, participants will be able to:
BPMH Training for Pharmacy TechniciansKimindra Tiwana, RPh, BSc. Pharm, ACPR Kimindra joined ISMP Canada in 2010. Her area of focus has been primarily in supporting medication reconciliation initiatives in Ontario and in the development of an iPhone app for patients to store and communicate their medication related information. Kimindra has been a practicing pharmacist since 2001. She received her pharmacy degree from Dalhousie University, College of Pharmacy and then went on to do a residency with the Queen Elizabeth Health Sciences Centre. The majority of Kimindra’s clinical experience comes from working at the University Health Network - Toronto Western Hospital.
