On Day 1, the virtual workshop will provide participants with background theory and hands-on practice in using a multi-incident analysis to analyze a group of medication incidents. On Day 2, the virtual workshop will provide participants with background theory and hands-on practice in using a novel tool called the Medication Safety Culture Indicator Matrix (MedSCIM) to assess and monitor patient safety culture within their organization

Day 1:
Multi-Incident Analysis - Analyzing medication incidents one group at a time

Program Abstract:

This workshop presents the key concepts for conducting a multi-incident analysis - a technique designed to maximize efficiency by analyzing a group of medication incidents sharing a common topic.

Participants will learn a stepwise process for conducting a multi-incident analysis, and situations where this methodology is most beneficial. You will have the opportunity for hands-on practice under the guidance of ISMP Canada experts. A variety of reference documents and supporting tools will be provided, enabling you to confidently analyze your facility's medication incidents.

Day 2:
Medication Safety Culture Assessment - A Novel Approach for Measuring Patient Safety Culture

Program Abstract:

This workshop teaches participants how to use medication incident reporting as an indicator to monitor patient safety culture within their organization. These concepts will be integrated to introduce a novel tool called the Medication Safety Culture Indicator Matrix (MedSCIM).

You will have the opportunity for hands-on practice to learn how to use Medication Safety Culture Assessment under the guidance of ISMP Canada experts. A variety of reference documents and supporting tools will be provided, enabling you to confidently use Medication Safety Culture Assessment to support a positive patient safety culture.

Time: Day 1: 9:00 am to 4:00 pm (Eastern Time)
Day 2: 9:00 am to 1:00 pm (Eastern Time)
Location: Join us virtually from your own location - virtual link will be emailed prior to the workshop
Cost: $850 per person, plus applicable taxes

In the Multi-Incident Analysis portion of the workshop, participants will learn:

  1. Describe how Multi-Incident Analysis complements other medication incident analysis strategies
  2. Describe the steps of the Multi-Incident Analysis process
  3. Apply the step-wise Multi-Incident Analysis approach in conducting an analysis on a small group of incidents

Day 2:
Medication Safety Culture Assessment - A Novel Approach for Measuring Patient Safety Culture

Learning Objectives:

In the Medication Safety Culture Assessment portion of the workshop, participants will learn:

  1. Understand the importance of patient safety culture
  2. Understand how analysis of medication incidents "by the numbers" can be misleading
  3. Understand the difference between quantitative and qualitative analysis of medication incidents
  4. Be able to use the Medication Safety Culture Indicator Matrix (MedSCIM) to assess organizational patient safety culture
